- Duly filled and signed application form.
- Recent coloured passport size photograph.
- Clear certified copy of University Degree Certificate.
- Clear certified copy of University degree transcript.
- Clear photocopy of Provisional Registration Certificate.
- Duly signed original internship completion forms in General Medicine, Paediatrics,General Surgery,Gynaecology and Obstetrics or Dentistry.
- Curriculum vitae.
- Proof of payment of prescribed fee.
Requirements for temporary registration.
- Duly filled and signed application form.
- Two (2) recent coloured passport size photographs.
- University degree/professional Medical or Dental qualification certificates.
- Transcripts of University degree/Professional Medical or Dental qualification Certificates.
- Current Certificate of Registration from Country of Origin.
- Certificate of Good Standing/Professional Status from Medical Council/Board.
- Letter of invitation/introduction from intended Ugandan employer.
- Curriculum Vitae.
- Three (3) reference letters from Professional Associates.
- For Specialists: Attach evidence of Postgraduate training three (3) years and above.
- All documents written in a language other than English MUST be accompanied by a copy of an official translation duly certified by a Notary.
